When query latency increases significantly (e.g., 20ms to 500ms) during peak traffic, the index may be approaching 90% capacity. This condition prevents new upserts from succeeding while queries continue to be served.

When vector database query latency increases, downstream services waiting for results may timeout, manifesting as 5xx errors in API gateways or service meshes. The vector database itself may be healthy while causing cascading failures.

Query latency consistently higher than expected (>150ms) when application and Pinecone index are deployed in different cloud regions or cloud providers, adding 50-100ms+ network round-trip time.

Slow data ingestion despite adequate capacity indicates inefficient upsert patterns. When uploading large volumes of data, single-record upserts or improper batch sizes cause throughput to be 10-20x slower than optimal.

Pod-based indexes at 90%+ storage capacity will begin failing upsert operations while continuing to serve queries. This condition requires vertical scaling (increasing pod size) which takes ~10 minutes but has no downtime.

Sudden increases in read unit consumption for serverless indexes can signal inefficient query patterns, such as high top_k values, unfiltered searches, or application-side query amplification requiring investigation before hitting rate limits.

For serverless indexes, exceeding per-second write unit limits causes upsert operations to fail with rate limit errors, blocking data ingestion pipelines and requiring retry logic with exponential backoff.

When query throughput plateaus despite increasing load, the index has reached its maximum QPS for the current replica count. Each replica adds ~25 QPS linearly, making this a predictable scaling mechanism for read-heavy workloads.
